New patients are asked to arrive 30 minutes early in order to complete new patient paperwork forms. A late arrival will result in rescheduling your appointment. Please be sure to complete these forms and bring them to your Pre-Visit Testing Appointment along with your insurance card(s), a photo ID and a list of your medications.
We understand that sometimes appointments need to be changed. We ask that you call in advance if you cannot keep your scheduled appointment. We require a 24-hour notice (1 full business day) so that we may accommodate the needs of another patient. Please be sure to review our cancellation policy in the new patient paperwork forms.
